A Primer on Anatomy, Physiology and Common Clinical Concerns

An integrative approach to Erectile Dysfunction:
Question: If he has obstructive sleep apnea (OSA) and ED, will fixing the apnea also fix the ED?
Answer: ED is multi factorial. If he has OSA, does he also have obesity, diabetes, hypertension, atherosclerosis and/or depression? Does he drink excessively or smoke? These conditions often go hand-in-hand.
It’s important to consider that maintenance of erection involves the circulatory system, the metabolic system, hormones and the mind. I would start with a set of labs and an evaluation of any meds he’s taking. Certain medications like antidepressants and antihypertensives (and alcohol!) can put a damper on male arousal.
A significant number of guys display ED as a first symptom of systemic cardiovascular disease so he might need to see a cardiologist before seeing a urologist.
The most powerful sexual organ is the one between the ears. If someone is experiencing stress at work, turmoil within their relationship or financial uncertainty, those situations can play a role in whether they can relax into a physical encounter. And if a guy is fixated on his inconsistent performance in the bedroom, ED can become a self-fulfilling prophecy.
Treating OSA makes a positive difference in overall health, but CPAP alone may not result in return to maintenance of erection.
